Please find the necessary forms for LIVING WATER COUNSELING.  If you are a first time client, please fill out the Welcome Client Info Form and the Service Agreement Informed Consent Form.  Your therapist will go over each of these in detail for you at your first meeting.  Feel free to give us a call if you have any questions!
